PRIVACY POLICY
1. DATA PROTECTION NOTICE
This Data Protection Notice (“Notice”) sets out the basis on which Smile Design Dental Practice (“we”, “us”, or “our”) may collect, use, disclose or otherwise process personal data of our patients and visitors in accordance with the Personal Data Protection Act 2012 (PDPA). This Notice applies to personal data in our possession or under our control, including personal data in the possession of organisations which we have engaged to collect, use, disclose or process personal data for our purposes.
2. PERSONAL DATA
As used in this Notice:
“Customer” means an individual who (a) has contacted us through any means to find out more about any goods or services we provide, or (b) may, or has, entered into a contract with us for the supply of any goods or services by us.
“Personal data” means data, whether true or not, about a customer who can be identified: (a) from that data; or (b) from that data and other information to which we have or are likely to have access.
Depending on the nature of your interaction with us, some examples of personal data we may collect include:
- Name, NRIC/FIN number, and contact information (address, email, telephone number)
- Nationality, gender, date of birth, marital status
- Medical and dental history
- Appointment and treatment records
- Photographs and other audio-visual information
- Employment information (if applicable for insurance claims)
Other terms used in this Notice shall have the meanings given to them in the PDPA (where the context so permits).
3. COLLECTION, USE AND DISCLOSURE OF PERSONAL DATA
We generally do not collect your personal data unless:
(a) it is provided to us voluntarily by you directly or via a third party duly authorised by you to disclose your personal data to us (your “authorised representative”) after (i) you (or your authorised representative) have been notified of the purposes for which the data is collected, and (ii) you (or your authorised representative) have provided written consent; or
(b) collection and use of personal data without consent is permitted or required by the PDPA or other laws.
We may collect and use your personal data for any or all of the following purposes:
(a) Performing obligations in connection with our provision of dental services requested by you
(b) Verifying your identity
(c) Managing appointments and treatment records
(d) Processing payments and insurance claims
(e) Responding to and processing queries, requests, complaints, and feedback
(f) Complying with applicable laws, regulations, and professional standards
We may disclose your personal data:
(a) Where required for performing obligations in connection with our provision of dental services requested by you
(b) To third-party service providers, insurance companies, laboratories, and agents engaged to perform functions listed in clause 5 above for us
The purposes listed above may continue to apply even in situations where your relationship with us has been terminated, for a reasonable period thereafter (including to enforce our rights under any contract with you).
4. WIFI USAGE
By using this service, you agree to the following terms of use Smile Design Dental Practice (SDDP)
4.1 Granting of Complimentary Shared Use
Smile Design Dental Practice (hereinafter referred to as “SDDP”) provides access to its internet services (e.g., Wi-Fi) to patients and visitors (hereinafter referred to as “Users”) free of charge and as a complimentary service. Use of this service is conditional and may be revoked at any time by SDDP. SDDP may limit or restrict the availability, speed, or use of the service at any time, either wholly or in part, particularly due to maintenance, misuse, or legal requirements. Access may also be restricted or denied if the service is used in a way that violates these terms, applicable laws, or the well-being of other Users.
Use of this service is not guaranteed and is offered on an “as is” basis. Access may be interrupted, suspended, or discontinued without prior notice.
4.2 Use of the Internet & Data Security
Please be aware that data transmitted over public networks, including this complimentary internet service, may be sent in unencrypted form. Therefore, SDDP cannot guarantee the confidentiality or security of any data while using this service. Sensitive transactions, such as online banking or password entry, are carried out at the User’s own risk.
SDDP shall not be held liable for any damages, data loss, malware, or unauthorised access resulting from the use of this service. Users are encouraged to ensure their devices are properly secured with updated software and antivirus protection before connecting.
4.3 Login Details
SDDP reserves the right to require login credentials for internet access. If login is required, Users agree to provide accurate and truthful information and not to share login credentials with others. SDDP may change or revoke login access at any time without notice.
4.4 Responsibilities and Prohibited Use
By using this service, Users agree to comply with all applicable local laws and regulations. The following actions are strictly prohibited:
- Using the service to access, transmit, or share illegal or harmful content
- Violating the rights of others, including but not limited to copyright, trademarks, or privacy rights
- Sending unsolicited messages (spam) or engaging in fraudulent or deceptive behaviour
- Attempting to access restricted systems or data without authorisation
- Engaging in activities that could harm, disrupt, or negatively impact the operations or reputation of SDDP
4.5 Data Logging
SDDP reserves the right to log, monitor, and analyse data traffic resulting from the use of this internet service, in compliance with applicable data protection regulations. Technical data such as connection times, visited websites, and device identifiers may be collected for service optimisation, security, and legal compliance.
Personally identifiable information will only be accessed or reviewed when legally required or necessary to ensure the safety or lawful operations of SDDP or others.
By using SDDP’s complimentary internet service, you confirm that you have read, understood, and agreed to these Terms of Use.
5. WITHDRAWING YOUR CONSENT
The consent you provide for the collection, use and disclosure of your personal data will remain valid until withdrawn in writing. You may withdraw consent by emailing us at info@smiledesigndental.com.sg.
Upon receipt of your request, we will require a reasonable time to process it and inform you of any consequences, including where withdrawal means we may no longer be able to provide services to you.
6. ACCESS TO AND CORRECTION OF PERSONAL DATA
You may request access to or correction of your personal data by contacting our Data Protection Officer (“DPO”) at the contact details below. A reasonable fee may be charged for access requests.
7. PROTECTION OF PERSONAL DATA
We take reasonable administrative, physical, and technical measures to safeguard personal data from unauthorised access, collection, use, disclosure, copying, modification, disposal, or similar risks.
8. ACCURACY AND RETENTION
We rely on personal data provided by you to be accurate and complete. Please inform us of any updates to your data.
We retain your personal data only for as long as necessary for the purposes for which it was collected, or as required by law.
9. TRANSFERS OUTSIDE SINGAPORE
We generally do not transfer personal data outside Singapore. If we do, we will seek your consent and ensure that your personal data receives a comparable standard of protection to that under the PDPA.

11. EFFECT OF NOTICE AND CHANGES
This Notice applies in conjunction with any other notices, contractual terms, or consent clauses related to the collection, use, and disclosure of personal data by us. We may revise this Notice from time to time without prior notice. The date of the latest revision will be indicated below.
